Five Strategies for Improving Therapy Documentation

For home health agencies, documentation can pose significant challenges to success. Not only is it a key driver in Medicare and Medicaid reimbursement, but it is pivotal to providing good patient care. In a recent review of over 1,500 records, Fazzi found that 80% could not support the total number of visits as being medically necessary to the patient. This amounted to a total of over 1.8 million dollars at risk.

Since HHCAHPS has gone into effect, agency leaders and staff continue to seek ways to  differentiate their service and improve their publicly reported HHCAHPS measures posted on Home Health Compare (HHC). But with over 30 questions on the standardized HHCAHPS survey, it is hard to know where to start

With over 1,000 HHCAHPS clients, Fazzi has reviewed over 1 million survey responses. The result? We have identified seven survey questions that show a direct correlation to scoring of the HHC global measures.
