OASIS Skills Assessment Testing

Fazzi's OASIS Skills Assessment Testing is designed to assess clinicians' OASIS skill levels on an individual and agency wide level. Developed by a panel of OASIS specialists who have over 50 cumulative years of experience working with OASIS, each quarterly test covers a wide range of critical OASIS assessment items. As an annual subscriber, your agency will receive:

  • Quarterly online OASIS testing for each registered clinician.
  • A one page summary report after each testing period outlining the score by discipline as well as the option to select two customized reporting categories (e.g., team, location).
  • A detailed report displaying results by individual clinician including a breakdown by question.
  • Immediate scoring e-mailed to the testing clinician and the agency manager after completion of each test.
  • Benchmarking externally against other agencies nationwide as well as internally to show improvements made and areas needing improvement. 

How it works

OASIS Testing Annual Subscriptions are designed to be implemented agency-wide.  Each quarterly testing session is held during a specific three week time period four times per calendar year (March, June, September, and December).  A copy of the OASIS Testing Schedule is available on the right side of the page. After registering, agencies will be sent a spreadsheet to submit the names of their clinicians and will automatically be enrolled in the closest testing session.

Fazzi’s new On Demand OASIS Skills Assessment Testing is the perfect tool for pre-screening potential staff or determining OASIS levels with new staff.  Available entirely online, this revolutionary system allows managers the ability to test and measure clinicians' OASIS knowledge anytime, anywhere.  Sold in bundles of ten, each OASIS Assessment package includes:

  • Anytime access to our online OASIS competency assessment covering the “must know” OASIS items – including those items that directly impact quality scores and financial outcomes.
  • Immediate scoring after completion of each assessment.
  • E-mailed reporting to the assessment manager, showing items responded to correctly and incorrectly as well as rationale for each item.
  • Certificate of completion with score for employee records.

How it Works

After purchasing the OASIS Competency Assessment package,  you will be set up with an account that includes a customized username and password as well as a secure OASIS assessment link to take the test.  Agencies will receive ten points for each assessment package purchased into their account.  When an evaluation is completed, a point is deducted from your account bank.  If you deplete your bank at anytime, you will always have the ability to add points to your account by purchasing additional packages.
