Therapy Documentation Audit
A sample of documentation from physical, occupational
and speech therapy is analyzed using the most recent
trends in denials as a guide to determine risk areas.
A detailed report is included outlining areas of concern,
quantifying the financial implications and providing
guidance as to “next steps”. A review sheet is attached
for each individual record in the sample. A conference
call occurs after the audit is complete to discuss the
findings and develop a follow up plan.
Reviewer Training for Therapy Documentation
Non- therapists are called upon to review therapy documentation quality and that can be a daunting task if the reviewer is unsure what to look for. One result is that documentation is only being superficially reviewed and when ultimately in the hands of others, a denial occurs. This session is specific to those that review therapy notes and teaches them exactly what to look for and how to approach therapy staff constructively to improve the quality of documentation.
