To be eligible to participate, agencies must:

  • Verify the support and commitment of senior management for the project
  • Name a lead person or “champion” for the project
  • Have case mix weights below the national average (SOC 1.31)
  • Be Medicare certified for more than five years
  • Have Medicare revenues equal to or greater than 50% of total revenue

OASIS Plus II Project

A National OASIS Improvement Project

Fazzi’s OASIS Competency Institute is seeking 50 agencies to participate in a national OASIS improvement project designed to help agencies to:

  • Improve your clinical outcomes
  • Improve your financial outcomes
  • Increase your case mix weight to at least the national average
  • Improve your clinicians’ OASIS accuracy

This 2011 OASIS Plus II Project, like our initial study launched in 2009, sets out to demonstrate that by utilizing a variety of learning opportunities over a sustained period, along with ongoing expert support, home health agencies will experience improvements in OASIS accuracy.  These improvements are closely tied to improved clinical and financial outcomes.

In our 2009 OASIS Plus project, 25 participating agencies started with case mix weights below the national average.  92% of agencies that worked on this with our industry experts achieved improvements in either or both clinical and financial outcomes.  In this second project, we plan to extend the reach of these positive results to 50 agencies with added experiences to promote success.

What Support Can Participating Agencies Expect?

The OASIS Plus II Project will be a collaborative vested project.  We are working under the assumption that if agencies are truly committed to improving and if they work with us and take full advantage of the ten month program with on-going support, they will have measurable improvement.

The OASIS Plus II Project includes OASIS skills testing of staff, live OASIS audits (with identification of OASIS discrepancies prior to submission), identification of specific areas that create challenges for staff and specific recommendations for improvement.  There will also be special web based trainings including monthly OASIS NP participation, manager coaching calls, and one-to-one consultant calls to hone in on agency-specific challenges.

OASIS TrackerOne of the unique components of the project is a track and trending report.  Fazzi will provide all participating agencies with a simple, automated track and trending software tool called the OASIS Tracker.  Using the same admission files that an agency already sends to the public health department, the Tracker will provide agencies with a monthly dashboard that tracks and trends CMW at RAP, revenues, primary diagnoses, and therapy utilization. These key metrics are then compared to national averages and other Tracker agencies. Each agency’s assigned senior consultant will monitor these reports and provide agency specific consultation and support when needed.

Cost and Other Considerations

By limiting participation, Fazzi Associates can provide senior level support to participating agencies while keeping the participation fee to a minimum. 

During the ten months of this project, there will be significantly reduced fees for the core services: training, monitoring, coaching sessions as well as a Fazzi senior consultant assigned to your agency ($2,000 for the ten month period) and a discounted “real time” OASIS audit cost of $150.00/record of 10% of your agency’s average Medicare census.  OASIS skills testing fees are included for up to 20 clinicians.  Beyond 20, the fee will be discounted to $40.00/clinician for the calendar year.  There are no additional charges for the Tracker and Trending services and your monthly OASIS Tracker dashboard.

The deadline for applying to participate is March 5, 2011.  The project will start on April 4, 2011.  There will be some pre-project activity during the month of March.
