OASIS Center of Excellence Certificate Program

Fazzi Associates is launching a new certificate program for participating OASIS Skills Assessment Testing agencies.  We are calling it the OASIS Center of Excellence.  It is our way of awarding agencies who achieve OASIS excellence recognition for their hard work and efforts. 

Agencies who achieve OASIS Centers of Excellence status will be part of a small but distinguished group.  Your agency will receive an honorary certificate from Fazzi's OASIS Competency Institute with recognition on our website and publications.  In addition, you will have access to exclusive webinar training sessions, VIP updates, and all OASIS Center of Excellence testing managers will be invited to join Fazzi's OASIS Competency Institute Review Panel.  

Requirements for participation:

1. Participating agencies must be current OASIS Testing Subscribers.  To order OASIS Testing, click here.
2. Agencies interested in becoming OASIS Center of Excellence should first complete the Intent to Participate form and submit to testing@fazzi.com or fax to 413-584-0220.
3. Participating agencies must subscribe to and test all clinicians within the agency who work with OASIS.  Agencies will be required to submit verification of number of clinicians prior to enrollment.
4. A 90% completion rate must be achieved for all testing cycles.
5. OASIS Centers of Excellence status will be awarded to agencies after 80% of those clinicians completing the test score 80% or higher, for three consecutive testing periods.
6. Each award is valid for one year.  Agencies wishing to renew may do so by continuing with the project.
7. Testing clinicians must take each test individually, and may not consult with other clinicians on testing questions during the testing cycle.
8. Participating testing managers must make all efforts to ensure that clinicians complete the test individually with only those approved materials.

There is no cost to participate.  This is a program developed to help recognize agencies who achieve OASIS success.
