The links below are Fazzi Associates’ Pay-for-Performance Home Health Compare Improvement Charts for each state. Agencies presently have their average “percentage score” for each Home Health Compare measure. The Fazzi Chart includes each state’s average percentage score for each measure, the national average for each measure, plus the estimated percentage score for each measure that an agency would have to reach in order to be in the top 20%. Being in the top 20% will most likely be the threshold that agencies need to reach in order to be eligible for a performance bonus under a Pay-for-Performance system.
For agencies interested in reaching the top 20% or better, this chart is your first step toward quality improvement. Whether you do it yourself or work with Fazzi Associates, start now! It will take six to nine months to begin showing improvement.
