Therapy
Practice in the Refined PPS Environment
September 7, 2008
Bloomington MN
The
Medicare Home Health PPS Refinements have significantly
altered the relationship between the provision of therapy
and home health reimbursement. The tiered model distributes
reimbursement over a larger range of therapy visits.
Therapists and those that supervise them need to have
a clear understanding of this system in order to ensure
positive clinical and financial outcomes. This session
will break down the tiered system and discuss the impact
it should (and should not) have on the provision of
therapy in home care. Case management with an intentional
focus on outcomes is a critical aspect of care delivery
that is often overlooked. Factors that influence clinical
decision making will be discussed and innovative treatment
approaches in key areas such as fall prevention and
reducing re-hospitalization will be presented.

The
focus on clinical outcomes continues to increase. Agencies
with "good scores" may not know what they
are doing well. To be successful, intentional interventions
that utilize the full scope of services with the agency
in a coordinated manner are critical. With limited financial
resources, attaining positive outcomes requires a focused
effort by the interdisciplinary care team. Being ready
for P4P will not happen by accident. Coordinated case
management has never been more important in home care. Faculty:
