Home Care OASIS-C Toolbox

OASIS data elements drive our home care industry. As part of a comprehensive assessment, OASIS-C data directs the patient Plan of Care, generates reimbursement dollars for the episode of care, reflects clinical practice, measures change in a patient’s health status with patient outcome and process measures and provides evidence of essential home health skills for competent providers.

The Home Care OASIS-C Toolbox includes the new and revised OASIS-C data elements. The toolbox contains three tools designed to improve clinical assessment practice, guide the SOC/ROC document review process and determine clinical competence with conducting a multi strategy assessment, applying the information correctly to the OASIS items and integrating it with the Plan of Care.

The OASIS WALK® is an interactive assessment strategy, focused on functional activity and the patient’s ability to interact safely in his environment given his current health status. This tool describes a process for conducting a functional assessment in an efficient manner and contains tips for selecting an accurate response for the corresponding OASIS item. It can be used for teaching and training related to assessment practices, mentoring, in orientation and for regular ongoing evaluation of clinical behaviors.

The Performance Based OASIS-C Competency tool is a companion for the OASIS-C WALK and is designed for use on site during a joint visit to determine if appropriate strategies are being used to gather assessment information and inter-rater reliability for OASIS-C data elements.

The OASIS-C Audit/POC Integration tool provides a guide for reviewing the consistency of information among the related OASIS-C items and between the comprehensive assessment and the Plan of Care. The tool is focused on reimbursement items and quality measures and can be used to objectively determine when a clinician’s documentation of SOC/ROC assessment and care planning no longer requires a 100% review.
