Tim Ashe, RN, MBA
Tim is the National Director of Operational Consulting
for Fazzi Associates. Tim brings in-depth, practical
understanding of home care and health care having
served as a registered nurse, charge nurse, clinical
nurse educator, nurse manager and senior hospital
administrator. He has served as project leader
for various
initiatives involving New York hospitals in the
Rochester area and has served as Assistant Professor
of Clinical Nursing for the University of Rochester,
School of Nursing. Tim has held the positions
of Director of Certified Home Health Agency Services
and Vice President of Clinical Operations for
Visiting Nurse Service of Rochester & Monroe
County in New York. Tim has served on committees
for the Home Care Association of New York and
NAHC and is actively involved in community events
and committees. |
Cynthia Campbell
Cindy is clearly a national home care leader.
She is a home health advocate at state and federal
levels; past NAHC Board Member and Chair of
the Pediatric Home Care Association
of America, current Executive Committee Member
for the American Telemedicine Association's
Special Interest Group: Home Telehealth and
Remote Monitoring. She is a highly skilled executive
change agent: has provided effective organizational
assessment, redesign and change management to
enhance efficiency and effectiveness in a variety
of home health settings throughout the country.

Dr. Robert Fazzi
Bob Fazzi is the founder and Managing Principal
of Fazzi Associates. Bob is a nationally recognized
speaker, author, futurist and change expert.
He is widely published in trade journals and
is the author of a number of books including
Management Plus™, a book on supervisory
management published by McGraw Hill and named
as a main selection on Fortune Magazine’s
Book of the Month Club. He has been Director
or Co-Director of many of Fazzi Associates national
studies and research projects and has provided
organizational, planning and change services
to hospital and home care agencies and associations
in nearly forty states.

Eileen Freitag, MBA
Eileen heads Fazzi's Strategic Consulting Division.
She is the past President and CEO of one of
the largest home care agencies in the United
States, the Boston VNA, has served as the CEO
of a long term care continuum that included
a major home health component, and the Vice
President of Business Development of a start-up
software company. During her 27 year career
in home care, she has served on the boards of
a variety of local and national health care
organizations. Eileen's division works with
home health agencies on a variety of marketing,
customer service, referral enhancement and interim
management efforts.

Lynn Harlow
Lynn is a Principal with Fazzi
Associates and manages the firm's administrative
and financial operations. She
directs our research on health care statistics
and has served as the Project Coordinator for
a number of Fazzi Associates National Studies
including the 3M National OASIS Integrity Project
and the National Operational Benchmark Study,
the project that led to the development of the
nation's largest best practice benchmark and
management system.

Nancy Kendrick, BSN, MPA
Nancy Kendrick represents excellence
in clinical and operational management. She
has
served in numerous senior management
positions in agencies throughout the country.
She has been a surveyor
for both CHAP
and JCAHO and has extensive experience and expertise
in regulatory compliance, risk management and
operational management. Nancy provides an array
of operational and clinical services as well
as Interim Management and compliance audits
for Fazzi Associates. Nancy is a true professional
who combines a quiet passion for home care and
hospice with a zealous commitment to excellence.

Cindy Krafft, MS, PT, COS-C
Cindy
has been in home health for 12 years in a variety
of capacities from visiting clinician to the
Director of Rehabilitation for a six agency
home care system. As a Senior Clinical Consultant
with Fazzi Associates, she brings tremendous
experience in rehabilitation. She has worked
with agencies on developing their rehabilitation
programs to their highest potential both clinically
and financially. Cindy has presented at both
state and national levels on topics of program
development, therapy utilization and recruitment.
Cindy is also the current Vice President of
the Home Health Section of the American Physical
Therapy Association and received her COS-C,
the designation of Certified OASIS Specialist
- Clinical. She is also an affiliate member
of the American Occupational Therapy Association.

Gina Mazza, RN, BSN
Gina
is the National Director of Fazzi's National
Patient Satisfaction Benchmark Service and
BestWorks®, our National Operational,
Financial and Quality Best Practice Service.
She has previously served in a number of senior
level positions ranging from Clinical Director
to QI/PI Director to Program Manager in two
of the largest hospital based home care agencies
in Massachusetts. She was inducted into the
Sigma Theta Tau International, Honor Society
of Nursing in May 2003 and named the 2004
Manager of the Year by the Home & Health
Care Association of Massachusetts.

Dr. Linda Scharf
Linda is truly a national expert when it comes
to the realities of running, managing and improving
home care agencies. She has had extensive administrative
positions in acute care and in home health agencies
and has served as an administrator for geriatric
service in a large health system. Linda has published
extensively and is an active researcher with special
interest in quality improvement initiatives including
OASIS, patient satisfaction, employee recognition
and patient outcomes. |
Dr. Carleton Townsend
Carl is a Principal with Fazzi Associates and
directs the firm's Quality Measurement division
and IS Operations. Carl is the primary architect
and developer of the Pat/Stat®
National Patient Satisfaction Benchmark System
and the primary developer of several of Fazzi
Associates major data systems including the
initial BestWorks® data system and Managed
Care Information systems. Carl is a trained
psychologist and researcher, and the author
of a number of articles on quality measurement
and patient satisfaction.

Rhonda Will, RN, BS, HCS-D, COS-C
When
it comes to
OASIS accuracy,
there is no one with
more expertise or better known than Rhonda Will.
She has over thirty years of nursing experience,
with twenty-five of those years in home health.
Rhonda was the Co-Director of the National OASIS
Integrity Project and is one of home care's
most well known national trainer and speaker
on OASIS. Rhonda heads Fazzi's OASIS auditing
team, a team of senior clinicians who have audited
over 2,000 patient records. Rhonda's focus is
on helping clinicians gain the skills, knowledge
and confidence needed to improve OASIS accuracy
and its ultimate impact on financial and quality
results.

Kay Wright, RN, MBA
Kay
is the National Director of Fazzi's Quality
Institute, a division dedicated to improving
the accuracy of agency OASIS assessments and
their quality outcomes. She has served as President
of a number of freestanding and hospital-based
organizations including one of New England's
largest home care agencies. Kay has served on
a number of national Boards and has directly
provided operational review services to agencies
throughout the country. She is recognized as
one of home care's premier experts on operations
and effective quality and financial management.

Kathy
Ziegler, BSN, MPH
Kathy
has had over thirty years experience in home
health. As President/CEO Kathy managed and directed
growth while building revenue and market share
for her agency to become one of the largest,
non-profit home health and hospice care providers
in the agency's city. She has also served on
numerous Boards, including the Georgia Nurses
Foundation, VNA of America, Robert Wood Johnson
Foundation and JCAHO.
