Fazzi Associates is the only home care consulting firm that continually conducts national best practice research studies while also having some of the oldest and most respected national operational, financial and quality best practice programs. We know what works and what doesn’t work and we apply these insights and learnings to every operational and strategic engagement we are involved in. These are services designed to help home care and hospice agencies improve their operations, quality and financial outcomes.

  • Quality Services The focus is on providing agencies with the tools to improve quality of services provided to each patient while also improving Home Health Compare and OBQI scores.
  • Compliance Services: For agencies concerned about adherence to conditions of participation, regulations, policies and procedures, clinical documentation and billing.

Best Practice Benchmarking and Research

Fazzi Associates has some of the oldest and most respected benchmark services. It is important to note that the focus of our benchmarking services is on making changes. What agencies are subscribing to are measurable improvements; we provide direct support. Fazzi views the benchmarks as critical tools essential to identify where changes are needed.

  • Best Practice Operational, Financial and Quality Benchmarking Services: The service is called BestWorks®. The focus is on helping agencies identify all the operational, staffing and practice factors that influence their financial and quality outcomes and then provide consultation, training and support to achieve measurable improvement in both areas.
  • Best Practice Patient Satisfaction Benchmark Services: The service is called Pat/Stat®. The goal is to provide agencies with JACHO and CHAP accepted surveys and reports that provide clear insights into what patients like and don’t like and how an agency can improve patient (and ultimately referral) satisfaction. Pat/Stat includes consultation, training and support to achieve measurable improvement.
  • Best Practice Staff Satisfaction Benchmark Services: The service is called Staff/Insights©. A nationally tested survey that segments responses of staff by position, by department or by any other way that you want. Includes consultation and specific recommendations by level for improving satisfaction and retention. Improvement services are also available.
  • Best Practice Referral Satisfaction Benchmarks Service: The service is called Referral/Insights©: This is a survey that provides agencies with a clear sense of what referral sources like about your services, what they don’t like and what they would like to see you do that would improve outcomes. Includes recommendations and strategies for improving relationships and increasing referrals.
  • National Research Projects: Many of the most recognized and most highly publicized research in the home care and hospice fields have been conducted by Fazzi Associates. They include our national OASIS Integrity Studies, Best Practice Operational Studies, Hospitalization Reduction Study and Future of Technology and Telehealth Studies. Fazzi has also partner with some of the most respected national and international companies in conducting these studies. Partners included the 3M Corporation, Philips, the National Association for Home Care and Hospice and the Briggs Corporation.
